Job Profile

JOB SUMMARY: The Chief Population Health Officer is an innovative and transformative senior leader responsible for the strategic development, design and implementation of a robust population health model encompassing value based care and coordination of care management. This leader possesses experience in building strong community relationships to foster strong, mutually beneficial partnerships as the foundation to a sustainable care model that addresses the comprehensive determinants of health. This leader will also develop and implement innovative chronic disease management strategies that positively impact outcomes in our community

MINIMUM QUALIFICATIONS:

Education: MHA, MBA, MPH, or related

Specialized Training/Licensure: M.D. or D.O. and Board Certified in a clinical discipline, with a license or ability to gain licensure in the State of Texas, preferred

Work Experience (Years and Area): 10 years of experience in public service with at least 7 years in healthcare

Management Experience (Years and Area): 5 years of experience in management/leadership role.

SPECIAL REQUIREMENTS:

Communication Skills: Above Average Verbal (Heavy Public Contact), Exceptional Verbal (e.g., Public Speaking),Writing /Composing: Correspondence, Reports

Other Requirements: An ambitious and successful senior leader with experience in academic and/or private medical settings and experience with insurance and managed care;
a. Record of successfully creating and translating strategic and operating priorities into implementation plans with clear accountability and monitoring of performance;
b. A creative visionary who understands the key trends in healthcare delivery, network development and technology, but also a practical tactician who knows how to implement sustainable change to position the organization to take advantage of those trends;
c. Ability to navigate ambiguous situations and effectively make decisions based on available information;
d. Demonstrated experience as an effective decision maker in an environment where engagement, inclusiveness and alignment with the medical staff is a priority;
e. Models strong, adaptive, and intuitive leadership and a responsive service culture across all parts of the Health System that involves nurses, AHP's, physicians, and employees at all levels and positions;
f. Highly credible and charismatic; understands how to lead a patient-centered and physician-led healthcare organization serving a diverse population;
g. Outstanding management skills with the ability to identify, recruit, train, develop, promote, and retain top talent;
h. Builds and leverages cross functional collaborative relationships to achieve shared goals;
i. Strong emotional intelligence and relationship orientation; ability to create a highly collaborative culture which enables effective communication and knowledge sharing, but also ownership and accountability for results;
j. Demonstrated commitment to service excellence and clinical quality.
