Outreach Worker - Healthcare for the Homeless - CDL Preferred

Performs a combination of activities including outreach, recruitment, care coordination, referral, and education, to engage homeless clients into accessing healthcare services.
Identify and assess new outreach sites and formulate implementation plans for service delivery
Conducts outreach services to address healthcare needs of target community
Serves as a community resource for patients, staff, and community partners
Delivers key health promotion messages to individuals and/or groups at multiple Harris Health System sites and/or offsite community locations throughout Harris County
Educates and engages homeless individuals to become Healthcare for the Homeless Consumers
Participates in health fairs and special events
Education: High School Diploma or GED
Licenses & Certifications: Basic Life Support from a hospital- based American Heart Association (AHA) approved program, CDL Certificate;Other certificates as required by unit/service preferred.
Work Experience: Two (2) yrs. experience in Public Health, Outreach, Social Work or related field.
Language Skills: Bilingual - Spanish preferred but not required

Work Schedule: Weekends, Flexible, Overtime, Travel
Other Requirements: Must be available by phone or pager after hours. Travel to outreach and worksites in the community. Able to work with little supervision, handle stressful situations and have excellent people skills. Must be able to interact successfully in a culturally diverse environment. Working knowledge of the Texas Medicaid Program, CHIP, and the public assistance benefit application preferred.
