Job Summary:

Transition & Post-Acute Care Pharmacy Manager priority is to deliver optimal medication management for Harris Health patients at care transitions, and post-acute care. Responsible for providing medication management services associated with care transitions with the purpose of improving health outcomes. Transition & Post-Acute Care Pharmacy Manager works in collaboration and partnership with Transition & Post-Acute Care (TPAC) services, as well as clinic, hospital, post-acute and insurance company partners, along with community resources, to achieve the desired patient care outcomes. Transition & Post-Acute Care Pharmacy Manager coordinates medication management services throughout the care continuum with the goal of enhancing patient health and well-being, improving adherence to medication programs, and reducing health care costs. This position will also be responsible for hospital at home programs including, infusion, OPAT, specialty referral medications need, and home infusion treatment.

The transition of care pharmacist manager role is to focus attention of medication management through the implementation and integration of innovative practices with the following goals: Improve access to medication, reduce adverse medication events, improve quality, prevent hospital readmissions, and reduce morbidity and mortality associated with the use of high-risk high alert medications in patient populations most vulnerable to harm.

Minimum Qualifications:

a.) Bachelors in Pharmacy
b.) Masters in PharmD. & Residency Required

Licenses & Certification: Pharmacist License (Texas State Board of Pharmacy)

Work Experience: Five (5) Years of Work Experience in Pharmacy preferably in inpatient and ambulatory setting.

Management Experience: Two (2) years of Management Experience in Management

Communication Skills:
a.) Above average Verbal Communication (Heavy Public Contact): Strong communication (written & verbal) and analytical skills, as well as the ability provide education
b.)Exceptional Verbal (Public Speaking)
c.)Writing/ Correspondence
d.)Writing/ Reports

Proficiencies: MS Access, MS Excel, MS Project, MS PowerPoint, MS Word
