JOB SUMMARY

The Service Linkage Worker (SLW) provides services within a broad range of focus areas to assist patients in accessing and adhering to care. Services provided may include HIV testing and counseling, mental health and substance abuse patient assessments, assisting to reduce access and adherence barriers, follow-up to ensure referrals are completed, patient navigation assistance, and coordination with case managers and other clinic staff.

Minimum Qualifications

Education/Specialized Training/Work Experience:
a) High School Diploma or GED (And) Five (5) Years Work Experience in providing client services to persons of a vulnerable population.
b) Bachelor's degree from an accredited college or university with a major in social or behavior sciences, or related health or human services field. (PREFERRED) (And) One (1) Year Work Experience in providing client services to persons of a vulnerable population.

Language Skills: Bilingual: Spanish (Preferred)
