Job Summary

Care Coordinator, LVN (CC-LVN) manages requests for Care Transitions & Integration by communicating with internal and external entities to ensure referrals are being completed. Facilitates authorization procurement for third party payers by providing pertinent clinical documentation according to department guidelines. Facilitates care coordination for complex patient care needs and assists with outpatient procedures and placement as indicated. CC-LVN keeps in communication with the continuum of care for the patient for their procedures and ensure they have a mode of transportation to attend appointments.  CC-LVN follows up with test results and ensures entry in the electronic medical record system for physician retrieval. Strong written and verbal skills are necessary in order to communicate with clinical staff, patient and vendors.

Minimum Qualifications

School Education: High School Diploma/ GED.

Licenses & Certifications: Basic Life Support (BLS), Licensed Vocational Nurse (LVN).

Work Experience: Three (3) Years of work experience in Physician Practice, Healthcare Experience, Case management experience.

Language Skills: Bilingual, Spanish, and Vietnamese (Preferred).

Communication Skills: Above Average Verbal Communication (Heavy Public Contact), Exceptional Verbal (Public Speaking), Writing/ Correspondence, Writing/ Reports.

Proficiencies: MS Access, MS Excel, MS PowerPoint, MS Word, PC.
