Job Summary

The Nurse Clinician, Home Division (NC-HD), has knowledge and skills necessary to meet standards for professional practice specified in Texas Nursing Practice Act and by Harris Health Nursing Service.

The practice patterns of competent generalists are characterized by the application of standards, principles, and algorithms to clinical situations. The Nurse Clinician- HD, is responsible for providing high-quality, acute care services within and beyond the four walls of a Harris health facility, including the patient's home. The areas of assignment are: (1) Hospital at Home (HaH) - inpatient (acute) care within the patient's home; (2) House Call Service (HCS) - outpatient (ambulatory) care within the patient's home; (3) Outpatient Parenteral Antibiotic Therapy (OPAT) - combination of inpatient and outpatient care coordination; and (4) Additional Services - other Home Division services as needed, based on patient care needs and organizational goals.  The NC-HD independently and safely manages patients with problems common to patients in his/her assigned clinical area; recognizes when he/she needs assistance and seeks appropriate resources.

The NC-HD, coordinates, facilitates, and participates in the identification, management, and education of patients suitable for management within the Home Division utilizing the appropriate eligibility criteria. The NC-HD develops, organizes, and facilitates the delivery of patient-centered care in the patients' home and/or a Harris Health facility by prioritizing patients' needs for in-home acute and ambulatory care delivery; utilizing a scheduling system for the efficient, timely, spatially and geographically-organized acute care home visits and point of care delivery; and accessing and using evidence-based resources and professional standards related to carrying out roles and responsibilities.

The NC-HD role models professionalism and promotes a customer-friendly environment by utilizing Service First behaviors while interacting with patients, families, and staff members.

Minimum Qualifications

Degrees:
- Bachelor's Degree in Nursing

Licenses & Certifications:
- Related Specialty: Other certificates as required by the Home Division
- Current Basic Life Support from an American heart Association (AHA)
or Red Cross approved program.
- Registered Nurse in the State of Texas or valid multistate/compact license
- Current and unencumbered Texas Driver's License

Work Experience:
- Three (3) Year Work Experience: One (1) year of experience in Home Health / Hospice OR Community visit setting (AND) Two (2) years of experience in an inpatient setting, specifically MedSurg or a higher acuity of care.

Language Skills:
- Bilingual (Preferred)
