This is an American Society of Health System Pharmacy (ASHP)-accredited (pre-candidate status) one year limited term position (July thru June).
The PGY2 Ambulatory Care Resident serves as an integral part of the multidisciplinary team and serves as the patient's advocate in addressing their pharmaceutical care needs. This is a 12-month ASHP-accredited postgraduate year two (PGY2) ambulatory care pharmacy residency position that offers individualized training and experience. The PGY2 Ambulatory Care Resident will build on pharmacy education and PGY1 pharmacy residency experience to develop independent practitioners that deliver state of the art clinical pharmacy services to ambulatory care patients. Skills are developed through supervised, direct patient care in pharmacist-managed clinics that provide comprehensive drug therapy management under a collaborative practice agreement approved by the Harris Health System medical staff. The PGY2 Ambulatory Care Resident will serve as a fundamental team member to continue to cultivate Harris Health System team-based care model initiatives while delivering innovative practices and service delivery models that ensure cost effective and optimal medication management in the population we serve.
MINIMUM QUALIFICATIONS
Degrees: PharmD from an ACPE accredited college of pharmacy.
Licenses & Certifications: Registered with Texas State Board of Pharmacy; If licensure by reciprocity or if new pharmacy graduate, must obtain Texas license within the first 90 days of employment.
Work Experience: ASHP accredited postgraduate year one (PGY-1) pharmacy
