Care Management Coordinator

Job Summary
The Care Management Coordinator (CMC) participates in the care management department with the guidance of the nurse care manager, social worker care manager, care coordinator, LVN, to facilitate patient transitions across the healthcare continuum; utilizes community and hospital resources to support continuity of patient care; and collaborates with medical staff, care management team members, business office, medical records and other Harris Health System staff to meet the needs of our patients. The CMC complies with quality standards and practices necessary to meet outcome targets for patient population served.
Minimum Qualifications
Degrees: Diploma / GED; Bachelors preferred
Work Experience: Three (3) Years of Experience in healthcare related field
Communication Skills: Above Average Verbal (Heavy Public Contact), Writing/ Correspondence, Writing/ Reports
Language: Bilingual Preferred; Spanish preferred
Proficiencies: MS Word, MS PowerPoint, MS Excel, MS Outlook, P.C., Typing (30-39 wpm)
