Case Management Coordinator

The Case Management Coordinator (CMC) participates in the case management department with the guidance of the nurse case manager, social worker case manager, navigator, and/or patient care coordinator to facilitate patient transitions across the healthcare continuum; utilizes community and hospital resources to support continuity of patient care; and collaborates with medical staff, case management team members, business office, medical records and other Harris Health System staff to meet the needs of our patients. The CMC complies with quality standards and practices necessary to meet outcome targets for patient population served.
Education/Specialized Training/Licensure: HIGH SCHOOL GRADUATE OR GED
Work Experience (Years and Area): 3 Years of Experience in healthcare related field
