Job Summary
The Clinical Nurse Care Manager (CNCM I) position supports the healthcare team in transitional care by being a liaison between patients, their families, doctors, and health care providers. CNCM I facilitates transitional care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction. In addition, the CNCM I helps to drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, process improvements, patient satisfaction). CNCM I is accountable for patient's caseload and plans effectively in order to meet patient needs across the appropriately deemed continuum of care, provide family support, manage the length of stay, and promote efficient utilization of resources. CNCM I advocates for patients and coordinates patient's care and work with other health care professionals to determine patient's goals and create both short term and long term plans. CNCM I participates in quality improvement activities, exemplifies professionalism, and promotes a customer-friendly environment by utilizing ServiceFIRST behaviors in interactions with patients, families, and staff members.

Minimum Qualifications

Degrees:
Graduation from an accredited school of Nursing with a Bachelor's degree required.

License & Certifications:
Register Nurse; Licensed to practice nursing in the State of Texas.
Related specialty certification preferred
Basic Life Support

Work Experience:
Three (3) years experience as RN in appropriate area assigned required;
One (1) year experience in Case Management, Quality Management, and Coding preferred.

Management Experience:
One (1) year leadership/clinical experience preferred.

Communication Skills:
Above Average Verbal (Heavy Public Contact), Exceptional Verbal (e.g., Public Speaking), Writing/ Correspondence, Writing/ Reports

Proficiencies: MS Word, P.C.
