Job Summary
The employee's number one job responsibility is to deliver optimal medication management across all dimensions of care every time. In this role, Clinical Pharmacy Coordinator role with Hematology/Oncology (Heme/Onc) will provide advanced clinical pharmacy services and coordination at Harris Health System. The coordinator will be self-driven and motivated to serve as a resource to the interdisciplinary team, and other pharmacists while delivering innovative practices and service delivery models that ensure cost effective and optimal medication management. The coordinator will serve as the residency program director, preceptor of pharmacy interns, technicians, and pharmacy residents. Each year, the coordinator will partner with the administrative director of clinical services & director of pharmacy to develop clinical initiatives. The coordinator will also collaborate with others to develop, coordinate, and evaluate target drug programs, restricted drug programs, drug usage evaluation programs, and cost containment program. The coordinator would be required to present to the Utilization Committee and Pharmacy Executive Leadership committee.
Minimum Qualifications
Degrees:
Bachelors degree in Pharmacy Required
PharmD, preferred
Licenses & Certification:
a. Registered with Texas State Board of Pharmacy; If licensure by reciprocity or if new pharmacy graduate, must obtain Texas license within the first 90 days of employment.
b. Completion of PGY1 from an ASHP-accredited residency program
c. Specialty residency (PGY2) in Oncology is strongly preferred
d. Board of Pharmacy Specialties in Oncology & ACPE IV Sterile Certification preferred
Work Experience: Five (5) years of total pharmacy experience including 3 years as a clinical pharmacy specialist in a progressive clinical practice in oncology required.
Management Experience: Two (2) years of related pharmacy management experience
Communication Skills: Above average Verbal Communication (Heavy Public Contact), Exceptional Verbal (Public Speaking), Writing/ Correspondence, Writing/ Reports
Proficiencies: MS Word, MS PowerPoint, MS Access, MS Outlook, PC
