Harris County Hospital District DBA Harris Health System seeks Clinical Pharmacy Specialist - CORHLT to work in Houston, Texas to perform direct patient care activities in the pharmacy, including prescribing (initiating, adjusting, discontinuing drug therapy) in accordance with the granted scope of practice. Perform clinical activities such as perform screening, verify medication orders, make drug therapy recommendations, monitor and assess medication effects, document clinical activities; report adverse drug reactions, conduct renal and hepatic screening, give P&T presentations, and perform medication reconciliation.
Engage in education/training activities, including educate physicians and nursing staff on medications and side effects; educate patients and caregivers and provide counseling about drug therapy, medications and side effect management; and provide training to pharmacy trainees (pharmacy residents and pharmacy students).
Provide clinical services to patients and professional staff. Interacts with medical staff on drug utilization and cost containment. Maintain competencies by attending staff development in-services and continuing education programs. Provide quality education regarding medication use to Harris Health System medical staff, pharmacists, pharmacy technicians, pharmacy residents and other allied health-care professionals.
Provide experiential and didactic educational programs to train the future generation of pharmacist.
Actively participates in clinical guideline development, critical pathways, disease management, pharmacy & therapeutics committee drug monograph review, MUEs, order set development, and drug use programs development. Contribute to research and development via publications and presentation. Mentor clinical pharmacists and residents. Other duties as assigned.
MINIMUM REQUIREMENTS:
MINIMUM EDUCATION: Bachelor's degree in pharmacy or Pharm. D. degree from an accredited school of pharmacy.
MINIMUM EXPERIENCE: Three (3) Years of progressive clinical pharmacist practice experience, or completion or PGY2 from an ASHP-accredited residency program
SPECIAL REQUIREMENTS:
ACPE IV Sterile Certification.
Board of Pharmacy Specialties Certification required or obtain within 18 months of employment.
American Heart Association Basic Life Support (BLS) certification within 90 days of employment.
Texas Pharmacist License or eligible for Texas Pharmacist License; must obtain licensure within 90 days of employment.
Completion of PGY1 from an ASHP- accredited residency program.
