The Clinical Pharmacy Specialist ' Care Transitions provides advanced pharmacy services for patients in OPAT and home-based care. This role focuses on optimizing antimicrobial and infusion therapies, ensuring safe transitions from hospital to home, and collaborating with interdisciplinary teams to enhance patient outcomes. The pharmacist also supports quality improvement, education, and mentorship within the department.

Job Summary

The Clinical Pharmacy Specialist ' Care Transitions provides advanced pharmacy services for patients in OPAT and home-based care. This role focuses on optimizing antimicrobial and infusion therapies, ensuring safe transitions from hospital to home, and collaborating with interdisciplinary teams to enhance patient outcomes. The pharmacist also supports quality improvement, education, and mentorship within the department.


Minimum Qualifications

  • Must have a Bachelor of Pharmacy or Pharm.D. degree from an accredited school of pharmacy

Licenses & Certifications

  • Texas Pharmacist License or eligible for Texas Pharmacist License; must obtain licensure within 90 days of employment.
  • Texas Pharmacist Preceptor Certification within 90 days of employment
  • ACPE IV Sterile Certification
  • Board of Pharmacy Specialties Certification required or obtain within 18 months of employment. Must maintain national certification such as Board of Pharmacy Specialties If certification is lapsed, employee has six months to rectify for new certification period.
  • American Heart Association Basic Life Support (BLS) certification within 90 days of employment.
  • Advanced Cardiac Life Support (ACLS) with course accredited by the American Heart Association (AHA) or per LRC will be required for inpatient positions prior to or within 6 months of employment
 

Work Experience

  • Completion of PGY1 and PGY2 from an ASHP-accredited residency program; OR
    PGY1 from an ASHP-accredited residency program with 3 years of progressive clinical practice within specialty area
 

Job Responsibilities

  • Provide comprehensive medication management for OPAT and home infusion patients
  • Optimize antimicrobial therapy and lead outpatient stewardship efforts
  • Collaborate with interdisciplinary teams for seamless transitions of care
  • Perform medication reconciliation and ensure accurate EHR documentation
  • Educate patients, caregivers, and healthcare providers
  • Support access to therapy through prior authorization and formulary guidance
  • Contribute to protocol development, quality improvement, and research
  • Mentor interns, technicians, and pharmacy residents
  • Ensure compliance with regulatory standards and institutional policies
