Job Description
The Health Care for the Homeless Program at Harris Health System began in 1988. It provides persons experiencing homelessness with healthcare, dental care, and mental health services. Each clinic location offers services such as healthcare visits for diabetes, high blood pressure, or other health related illnesses. The clinics offer immunizations, yearly physicals, HIV/Hepatitis C testing and other blood tests. At each location, a person experiencing homelessness may find help from social workers or case managers who can assist with transportation, employment, housing, identification and mental health services.

Job Summary
Position responsible for participating in the coordination of all systems / services required for an organized, multidisciplinary team approach and assures quality, cost-effective care for the identified patient population
Healthcare for the homeless is seeking a clinic therapist to support the team in:
1. Assistance with, and follow-up of, patients in a specifically designated area, serving specific population,
under supervisory guidance.
2. Completes assessment and re-assessments pursuant to identified needs.
3. Provides therapeutic intervention that promotes health goals and/or continuity of care.
4. Provides follow-up and service planning.
5. Documents consistent with HCHD/Departmental standards.
6. Participates in intra-departmental committee work, team conferences, etc., as assigned.
7. Maintains accurate, up-to-date documentation in the medical record, departmental records, and
computer.
8. Participates in data collection and research activities related to assigned areas.
9. Assists in transportation arrangements / issues of clients.
10. Assists with information and referral services.
11. Identifies and reports quality assurance issues.
12. Participates in staff meetings and self-directed work teams and other department initiatives for
departmental improvement.
13. Actively participates in training and development as recommended by Director.
14. Adheres to guidelines and requirements set forth by HRSA Ryan White.
15. Other duties as assigned.
Minimum Qualifications
Degrees:
Masters Degree in Behavioral Sciences or Social Work with Clinical Licensure.
Bachelors of a Accredited Program
Licenses & Certification:
Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC) or other License Clinical Counseling Certification; Texas State Board Certification
Work Experience:
One (1) Year Work Experience in Case Management, Quality Management, or Discharge Planning preferred.
Two (2) Years Work Experience of social work/therapist experience in a mental health setting is required
Management Experience:
One (1) Year of Management Experience of leadership experience preferred.
Communication Skills:
Above average Verbal Communication (Heavy Public Contact), Writing/ Correspondence
Language:
Bilingual Skills; Spanish preferred
Proficiencies:
MS Word, MS Outlook, P.C.
