Skills / Requirements

The Community Health Worker (CHW) will serve as an integral member of Harris Health's interdisciplinary

care team, partnering to help patients improve their health holistically. The CHW will work directly with

patients to assess needs across all aspects of their lives, including economic and social challenges

alongside their physical and mental health needs. The information gathered will support the development

of a team-based action plan, and the ongoing work with the patient will include establishing the patient's

priorities and working together on goals. The CHW will be the point person for a panel of pregnant and postpartum patients,

available and accessible to provide assistance. They will work alongside, and will be supported by, a care

team associated with each patient in order to help improve patients' total health.

 

The Maternal Substance Use CHW will work in partnership with the maternal substance use program at Ben Taub to ensure patients are supported during and after pregnancy and throughout their treatment for SUD. Candidates with experience with vulnerable populations, harm reduction, community outreach, and/or substance use disorder treatment are ideal candidates. The CHW will collaborate with the clinical team to engage pregnant and postpartum women with SUD in treatment and support their care delivery. The position involves the inpatient, outpatient, and community setting.

 

 

The CHW will support patients with services that may include but is not limited to Eligibility Screening, Health Insurance Enrollment, Prevention Screening, Coordinating Special Projects,  Lifestyle Modification, Health Coaching, and Substance Use Treatment and Prevention Services. Additionally, responsibilities to include coordinating special projects, leading team meetings, conducting peer chart reviews and participating in team member trainings.

 

Minimum Qualifications


Degrees / Work Experience / School Education: Bachelor's Degree in health services, public health, sociology, psychology or related field (And) Three (3)Years Work Experience with patient navigation, care management, or any kind of direct service provision

to patients in a community health/outreach, and/or health care setting plus a bachelor’s degree in health

services, public health, sociology, psychology or related field

 

(Or)

 

High School Diploma or GED (And) Seven (7) Years Work Experience with patient navigation, care

management, or any kind of direct service provision to patients in a community health/outreach, social

resource agencies and/or health care setting

Licensure/Certification:
Current Texas DSHS CHW Certification

Special Requirements
Communication Skills: Above Average Verbal (Heavy Public Contact).
Writing /Composing: Correspondence, Reports.

Other Skills: Medical Terms, PC, Word Proc., Epic

Language- Spanish (preferred)

 

Knowledge/Skills/Abilities

Analytical Abilities: Electronic Medical Record- Average skills achieved upon training.

 

Other Special Requirements

The CHW must have access to reliable transportation to travel to worksites throughout

Harris County.

Advanced Education:
Advance Training-Certification in related specialty area within two years of employment.

Work Schedule: Monday- Friday, 8:00am- 4:30pm, Travel.
