Skills / Requirements

Provides direct services to CHC Members and their families for purposes of culturally appropriate health education, navigating healthcare systems, identifying available resource assistance and certification for SSI if applicable.  Works closely with case management, DM programs and other initiatives of Health Services and CHC.
Minimum Qualifications
Education/Specialized Training/Licensure: BA preferred or 3 years managed care experience, CHW certified or obtained within 1 year of employment.

Work Experience (Years and Area): 3 years managed care  ¿customer service, CHW experience

Equipment Operated: Must be PC literate, fax machine, printer, phone , copier

Special Requirements
1. Communication Skills: Above Average Verbal (Heavy Public Contract), Exceptional Verbal (e.g., Public Speaking), Writing /Composing   (Correspondence/Reports)

Other Skills: Medical Terms, P.C., MS Word, MS Excel

Weekends 1     Holidays    Flexible 1       Overtime 1       Travel 1       On Call 0

Other Requirements: Microsoft Office Suite, filing, telephone skills, traveling in personal car, working independently. Bilingual (English and Spanish) strongly preferred
