JOB SUMMARY

The Social Determinants of Health, Dietician role will provide dietary counseling according to health-related conditions that aids in the improvement of members' health outcomes. This role will create meal plans tailored to the members dietary restrictions and requirements. Additional responsibilities consist of collaborating and establishing short and long-term health goals with members; while teaching healthy nutrition options, developing effective exercise plans guided by the members physician supported care plan.


JOB SPECIFICATIONS AND CORE COMPETENCIES

  • Educate, advise, and guide members on nutrition plans that help them maintain a healthy lifestyle, improve health outcomes, and manage chronic conditions.
  • Conduct initial welcome calls to members with nutrition and food needs and complete assessments to determine dietary restrictions, nutrition needs and cultural/religious perspectives on nutrition and food.
  • Collaborate with interdisciplinary teams to improve health outcomes of members, including collaboration on workflow development and creation of care models.
  • Tracks overall metrics and social needs of members including specific nutrition and food insecurity related data of members.  Make referrals to community programs as needed.  Collaborate with SDOH related staff to address social needs outside of nutrition and food insecurity.
  • Support nutrition and food insecurity related partnerships.  Create and maintain a repository of nutrition related partners and resources provided.
  • Complete in-person and virtual trainings related to all Community Health Choice plans, social needs, nutrition and service coordination.
  • Participate in curriculum development of Service Coordination training and assist with trainings related to nutrition and social needs.
  • Actively contributes to achievement of departmental goals, as identified in Departments annual business plan, including specific departmental process improvement plans.


QUALIFICATIONS:

  • Education/Specialized Training/Licensure: Bachelors Degree and Registered Dietician or Registered Dietician Nutritionist required.
  • Work Experience (Years and Area): 3 years' experience counseling members on maintaining a healthy diet required.
  • Software Proficiencies: Proficient in MS Office (Word, Excel, Outlook)
  • Other: Experience with Medicaid populations and/or underserved communities required.
  • Texas Medicaid Experience (STAR/STAR Plus), Medicare and Market Place experience preferred.
