Job Summary
The Health Lifestyle Coach provides nutrition education and lifestyle coaching to patients, working closely with the multidisciplinary healthcare team to help improve health outcomes for patients diagnosed with lifestyle-related chronic diseases related to diet and physical activity. Primary responsibilities include providing classes and individual coaching sessions to patients in-person or through telehealth that are evidence-based and patient-centered, managing the daily schedule to maximize patient care. Additional responsibilities may include assisting with projects, presenting on services to the healthcare team, and training new staff.
Minimum Qualifications
Degrees:
Bachelor's Degree in a health related field required (Degree in nutrition or dietetics preferred)
Licenses & Certifications:
Certified as a health or wellness coach through a National Board of Health and Wellness Coaches (NBHWC) approved program; required within six (6) months if held at time of appointment.
Work Experience:
One (1) year work experience providing nutrition education and/or lifestyle coaching to patients or in the community preferred
Language Skills:
Spanish skills preferred

Job Competencies:
All required job competencies need to be completed within 90 days of hire.
Advanced degrees:
Masters in Public Health, Nutrition, or a related health field may count for one year of experience.
