Skills / Requirements
The Heart Failure Nurse Navigator (HFNN) coordinates the care of identified patients with Heart Failure in conjunction with the patient, primary care team and cardiology team, according to agreed-upon clinical guidelines, with the objective of coordinating high quality patient care throughout the hospital stay. The HFNN collaborates with case manager and social worker on identifying expected post-acute care needs. The HFNN collaborates with Cardiology Services, Ambulatory Care Services (ACS) and other departments throughout Harris Health System to ensure heart failure patients have full, equitable and convenient access to the clinical services they require and to the cardiac ' sensitive health education information needed to make informed decisions about their care.
The HFNN participates in quality improvement, best practices, research processes/utilization and community activities. The HFNN role models professionalism and promotes a customer ' friendly environment by utilizing ServiceFirst behaviors while interacting with patients, families, and staff members.
1. Education/Licensure/Specialized Training:
a. Education: Graduation from an accredited School of Nursing with a Bachelor's degree in Nursing.
i. Licensed to practice nursing in the State of Texas.
ii. Related specialty certification required within two (2) years of employment.
2. Work Experience:
i. Three (3) - five (5) years of clinical nursing experience.
ii. Two (2) years minimum experience in the care and management of target patient populations specific to area of employment as inpatient/outpatient cardiac care staff nurse.

4. Equipment Operated: PC, computer tablet and typical medical equipment related to job duties.
