Job Profile

JOB SUMMARY:  The HPV Self Testing Care Coordinator shall assist the Mammography Program Management senior nurse case manager and/or senior care coordinator in maintaining the cervical cancer screening test database for patients who rarely or have never been screened and encourage self-testing of HPV for cervical cancer screenings at home. Additional responsibility includes the scheduling of follow-up appointments for additional diagnostic testing as required for all facilities, track all interventions, and assist with education and outreach activities. The HPV Self Testing Care Coordinator will utilize the American Society for Colposcopy and Clinical Procedures (ASCCP) guidelines within the Breast and Cervical Cancer Services (BCCS) grant and use them accordingly. The HPV Self Testing Care Coordinator will accurately document in the grant database and Electronic Medical Record (EMR), as well as train and properly use the Oncore database.  The HPV Self Testing Care Coordinator will also be familiar with the recruiting script and stay within the approved verbiage, verify patient demographic prior to mailing kits and process to lab once received.  The HPV Self Testing Care Coordinator shall also verify compliance with financial and other eligibility requirements as outlined by the funding sources Chassis, Centers for Disease Control (CDC), Medicare, and Medicaid etc. 


Degrees: High school Diploma or GED

Work Experience: (Five (5) or more years in a primary or specialty health care setting possibly including experience in medical records, appointment center and eligibility. 

Communication Skills:
Above Average Verbal (Heavy Public Contact)  
Bilingual Skills Preferred.
