Skills / Requirements

JOB SUMMARY: The HPV Vaccine and Vaping Prevention Care Coordinator Data Specialist shall assist the Mammography Program Management senior nurse case manager and/or senior care coordinator in maintaining the Pediatric HPV Vaccine and Vaping Database. Additional responsibilities include scheduling of follow-up appointments for additional vaccines or assessment for nicotine use and assisting with education and outreach activities. The Care Coordinator Data Specialist shall also verify compliance with financial and other eligibility requirements as outlined by the funding sources Chassis, Centers for Disease Control (CDC), Medicare, and Medicaid, etc.

MINIMUM QUALIFICATIONS:

Degrees: High school Diploma or GED

Work Experience: Five (5) or more years in a primary or specialty health care setting possibly including experience in medical records, appointment center and eligibility.

Proficiencies: MS Word, P.C., Typing 45_WPM
