Job Summary
The Manager, Care Management Education and Coaching, reports to the Director, Care Management Education and Coaching, and serves in a leadership role to ensure the provision of exceptional care management services across the continuum of care. This role is responsible for designing, evaluating, updating, and implementing new and current care management educational curricula, including concepts in care management foundational principles, crisis intervention and management, relationship building, multi-cultural aspects of care, working with special populations, complex care coordination skills and interventions, and disease-specific best practices.
The Manager is responsible for the development and maintenance of competence and evidence-based resource material for staff and ensuring staff's understanding and content retention through the coordination, oversight, and facilitation of educational in-services, orientation programs, and coaching sessions. The role fosters staff development of professional practice behaviors and adherence to the Case Management Standards of Practice and Scope of Services. In addition, this role participates in ongoing quality improvement activities, provides support to the department's leadership team, contributes to policy development, collaborates with affiliate organizations as appropriate, is an active participant on various committees and workgroups, is consistently on-site at Pavilions for observation and coaching sessions, and provides education to the larger organization regarding Care Management topics.

Minimum Qualifications
Degree: Master's in Nursing or related field (Healthcare Administration, Business Administration, or Public Administration); or Social Work

Licensure & Certification:
Licensed to practice professional nursing in the state of Texas or Licensed to practice professional social work in the state of Texas: LMSW, LMSW-AP, or LCSW.

Accredited Case Manager certification (ACM or CCM): To be completed within two (2) years if not held at the time of appointment.

Work Experience: 5 Years of Experience working in a challenging healthcare environment. Experience designing, evaluating, updating, and implementing new and current care management education curriculums preferred. Inpatient medical/surgical, intensive care unit, utilization review, and ambulatory clinic experience preferred.

Management Experience: Two (2) years of experience related management or leadership experience required. Specific experience in managing care management operations in ambulatory and acute care settings including oversight of multidisciplinary teams and collaborative coordination across the continuum of care preferred.

Communication Skills: Above average Verbal Communication (Heavy Public Contact), Exceptional Verbal (Public Speaking), Writing/ Correspondence, Writing/ Reports

Proficiencies: MS Word, MS PowerPoint, MS Excel, MS Outlook, P.C.
