Skills / Requirements
Position responsible for participating in the coordination of all systems / services required for an organized, multidisciplinary team approach and assures quality, cost-effective care for the identified patient population.
Education/Specialized Training/Licensure: Masters Degree in Behavioral Sciences or Social Work with Licensure (LPC, LCDC, LMSW).
Work Experience (Years and Area): 3-4 year experience in a hospital/health setting preferred
Experience working with patients with chemical use/abuse/dependency counseling and treatment preferred.
