Job Summary

The Manager, Care Management Program Operations (MCMPO), serves as a critical operational liaison and strategic partner to the Vice President, System Care Management, ensuring alignment, coordination and optimization across all System Care Management departments (>250 FTEs) within Care Transitions and Integration (CTI). These departments include, but are not limited to, Inpatient and Ambulatory Care Management teams, Outsourced Medical Services, and Utilization Management, as well as operations related to all care management for special programs. The MCMPO is responsible for planning, implementing, reviewing and controlling operational projects, process improvement initiatives and business development engagements as defined by CTI leadership. This role is directly responsible for the maintenance of operational targets as established and is expected to work with multiple stakeholders across the health system to meet business goals. The MCMPO will be charged with continuous improvement of the
operations of each department, focused on data, operational effectiveness, and identifying areas of opportunity.

The MCMPO will play a key role in working with the VP, System Care Management to ensure alignment across all care management departments across the system, developing operational communication tools, standardizing and improving processes, and supporting system-wide initiatives that include care management as a key stakeholder. This role will also be responsible for providing critical administrative support such as the scheduling and coordinating of meetings for Care Management leadership, the documentation and management of significant meeting discussions, tracking of relevant action items and ensuring adequate supply monitoring and ordering for CTI leadership and staff for business continuity, as required. This role will be a liaison and work in partnership with CTI Business Operations for all data and finance, education, and project management needs for System Care Management. 

This role should be proficient in the ability to manage resources and routinely perform continuous quality improvement activities that align with the strategic vision for the care management operations alongside the System Care Management leadership. This role must have demonstrated ability to work independently and execute multiple initiatives concurrently and manage other duties as assigned.

Minimum Qualifications 

Degrees/School Education: 
- Master's Degree: Nursing, Healthcare, Business or Public Administration, Social Work, or any other business or healthcare related degree
- Bachelor's Degree
- Associate's degree
- Diploma / GED

Licenses & Certification
- Lean Six Sigma Green Belt: Obtained upon hire or within 2 years of hire

Work Experience:
- Four (4) Years Work Experience in a complex healthcare environment 

Management Experience: 
- Two (2) Years of Management Experience of leadership or supervisory experience 

Communication Skills:
- Exceptional Verbal (Public Speaking)

- Above Average Verbal Communication (Heavy Public Contact)
