Job Summary

The Outreach Worker (ORWD) will serve as an integral member of Health Care for the Homeless enabling services team, to conduct in-reach, outreach, street medicine, and education activities to existing health center patients and non-health center patients within the Homeless community. The ORWD will work directly with patients to assess needs across all aspects of their lives, including economic and social challenges alongside their physical and mental health needs. The ORWD will be the point person for consumers within the HCHP targeted population and will be supported by, a care team associated with each patient in order to help improve patients' total health.

Minimum Qualifications

Education/Degree:
a) High School Diploma or GED
b) Associate's Degree: in a Health Care related field, from an accredited
college or university preferred (Preferred)

Licenses & Certification:
a) Texas State Driver's License: CDL Certificate
b) Basic Life Support: Basic Life Support from a hospital- based American Heart Association (AHA) approved program

Work Experience:
a) Two (2)Year Work Experience: Demonstrated history in operating large commercial vehicles (Preferred for Mobile Medical Fleet Outreach assignments)
b) Two years of experience as a CDL qualified driver in lieu of education requirement

Language:
a) Bilingual (Preferred): Spanish preferred but not required

Communication Skills:
a) Above average Verbal Communication (Heavy Public Contact)
b) Exceptional Verbal (Public Speaking)
c) Writing/ Correspondence
d) Writing/ Reports

Proficiencies:
