Job Description

Skills / Requirements
Job Summary
Establish a liaison relationship with patient/families, community partners, and significant others for needed services. Make continued rounds at the clinic to receive referrals from the staff. Facilitate the process for inmate care concerns through collaboration with all Correctional Health departments and the Harris County Sheriff's Office. Participate in rounding completed on incarcerated patients located in inpatient Harris Health hospitals.
Minimum Qualifications
Degrees:
a) Masters in Social Work
Work Experience:
a) Three (3) years of work experience: as a patient advocate or social worker.
Communication Skills:
a) Above Average Verbal Communication (Heavy Public Contact)
b) Exceptional Verbal (Public Speaking)
c) Writing/Correspondence
Proficiencies:
a) MS Word
b) Personal Computer
