Job Summary - CORRECTIONAL HEALTH

The Patient Navigator - EC Frequenters coordinates comprehensive patient care across the healthcare continuum and provides individualized support to patients as their resource and advocate. The navigator collaborates with other disciplines and departments throughout Correctional Health and ensures that patients have full, equitable and convenient access to clinical services.

The navigator performs program related functions. The navigator also provides direct clinical nursing care within her/his scope of practice, education, and training; and participates in performance improvement, research processes/utilization and community activities. The navigator role models professionalism and promotes a customer-friendly environment by utilizing ServiceFirst behaviors while interacting with patients, families, and staff members.


Minimum Qualifications

Degree: Bachelor's degree in Nursing

Licensure/Certification:
Licensed to practice nursing in the State of Texas.
Related specialty certification required within two years of employment.

Work Experience:
Three (3) - five (5) years of nursing experience
Three (3) years experience as patient navigator or related role in patient referral, case management, public/private clinic preferred.

Communication Skills:
Above Average Verbal (Heavy Public Contact)
Exceptional Verbal (e.g., Public Speaking)
Bilingual Skills Required: No; Language(s): Spanish preferred
Writing /Composing (Correspondence/Reports) including entry of case management data

Proficiencies: P.C.,Word Processing
